The Dental Office of Matthew R. Lark DDS MAGD and Edward J. Jacob DDS is a Frank Lloyd Wright Style Building residing at 4315 N. Holland-Sylvania Rd.,Toledo, Ohio 43623 Phone: 419-824-7900.

This general dentistry practice provides a full range of dental services for patients, including Aesthetic Dentistry, Implant Placement and Restoration, comprehensive Caries (decay) Management, and non-surgical Periodontal Therapy. The office has a state of the art Dental CAT Scan imaging unit which aids in diagnosing complex TMJ, Oral Pathology, and is used in treatment planning for Implant Dentistry.

Dr. Lark is a 1981 Graduate of the University of Detroit and has earned his Masters in the Academy of General Dentistry.

Dr. Lark is also Board certified in Orofacial Pain and Temporomandibular Disorders and has received a certificate in Surgical Placement of Implants from the Misch Implant Institute. He is also a Clinical Instructor at the prestigious Kois Center of Seattle where he teaches courses on Occlusion, Restorative and Implant Dentistry, Biomaterials, and Periodontics.

Dr. Jacob has been in General Dental practice for over 35 years in West Toledo and joined Dr. Lark's team in November 2005.

The practice hours are 8:30- 5:00 M,T,TH, and 8:30-1:00 W F. We are accepting new general dentistry patients, patients with Implant needs, and referrals for TMJ or Facial Pain. We accept most Dental Insurance and have flexible payment plans and accept Visa, Discover , and Mastercard.
